Once you have been selected to join our team, we will need you to fill out the following forms for legal reasons. In addition, we will also need a copy of your child's birth certificate and a head shot. You can send these documents to: info@iamimpactfc.com
This is an Emergency Medical Release & Liability Waiver and registration for the Missouri Youth Soccer Association (MYSA) and US Youth Soccer (USYS). There is a $19 fee associated with this registration.
